Installed the plugin but doesn't show up anywhere. Not even a mu item for it. NOTHING. Not much more I can say because I can't even use it to test it. Maybe considering updating it once every few years?

I made a beautiful form for donation to a not-for-profit and it disappeared! It literally VANISHED! I bought the plugin through THEIR website and was given no way to enter a license number. I am really furious since it took me a lot of time to construct the perfect form AND NOW IT IS GONE!

I have used this plugin and I think that it has been very helpful to me. I like the way it integrates with another plugin by the developer, Smart Forms. At first I was confused, but after I have figured it out, I decided that its really neat. I also received good help from the developer. Thanks
